If you require an urgent appointment, please call the service directly. To make an appointment with a physician, you should call the physician’s office.
If you are experiencing a medical emergency, call 911 or visit your nearest emergency room.
This form only allows you to request an appointment. The registrar who receives your request will call you to confirm a date and time. Please have your doctor’s prescription and your insurance cards available when the registrar calls. We will do our best to accommodate your requested dates and times.
