Comfort and Care at the End of Life
Since our founding in 1983, we at Phelps Hospice have provided a special kind of care for our patients and families as the end of life approaches. We provide care in the familiar surroundings of “home,” which may be the patient’s own home or that of a relative or, in some instances, a nursing home or an assisted living facility located anywhere in Westchester County.
We adapt our services to meet the unique physical, emotional, and spiritual needs of each patient and family, providing comprehensive support and professional guidance through this difficult time.
Phelps Hospice cares for patients who have cancer or advanced respiratory, cardiac, liver, or kidney diseases, as well as those who have Alzheimer’s, Parkinson’s, AIDS, ALS (Lou Gehrig’s Disease), or other life-limiting illnesses.
Ensuring Quality of Life
A person's quality of life can be diminished by the suffering caused by pain. Physical pain. Emotional pain. Spiritual pain.
Phelps Hospice seeks to alleviate the pain in different ways. We offer the most advanced pain medications available, and we encourage our patients to be partners with us in the management of this pain.
Importantly, we also provide counseling and other supportive services to relieve the emotional or spiritual pain that may exist for our patients and their loved ones.
Receiving the Best Possible Care
Most of our patients are referred to us by their physicians. We also welcome referrals from family members, friends, members of the clergy, or prospective patients themselves.
Upon admission to our program, the Phelps Hospice interdisciplinary team, in consultation with the patient’s primary physician, evaluates the patient’s and family’s needs and develops an individualized plan of care which provides medical care and supportive services. Our goal is to enhance the quality of life for the time remaining.
The Phelps Hospice interdisciplinary team consists of the medical director, hospice director, patient care coordinator, nurses, home health aides, social workers, consulting psychologist, nutritionist, chaplain, bereavement coordinator, and volunteer coordinator.
As patients and families face difficult decisions, Phelps Hospice staff members bring the knowledge and experience gained in assisting other families for more than two decades. We may serve as consultants, guides, or simply compassionate listeners.
A Phelps Hospice nurse is on-call 24 hours a day, seven days a week, 365 days a year, to answer questions and offer support to our patients and family members.
Sometimes a patient requires short-term hospital care. In that event, the Hospice staff and the patient's primary physician oversee the patient's treatment in the hospital.
Phelps Hospice Offers a Range of Services
Home care services for each patient may include:
- visits by a registered nurse
- instruction and assistance in pain management
- prescription medication for symptom control
- assistance with personal care
- counseling by a social worker
- spiritual counseling
- special support from trained volunteers
- physical, occupational, speech, and respiratory therapy
- nutritional guidance by a registered dietitian
- medical supplies and equipment
Bereavement Support
Phelps Hospice offers a program of bereavement support services that provide guidance through the journey of grief. All our bereavement services are available to the community, as well as to our Hospice families and friends.
What Does Hospice Care Cost?
Hospice care is covered by Medicare, Medicaid, and most private insurance plans. Phelps Hospice is certified by the State of New York and accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). We provide services to all patients regardless of race, color, religion, sex, age, physical or mental handicap. Our services are not denied for financial reasons.
Since the traditional methods of payment do not cover the total cost of operating Phelps Hospice and its special programs, we encourage our friends and supporters to help us through private and corporate contributions.
